Care is based on the person’s needs and the tasks that can be provided safely. Brief check-ins may be useful in some situations, but the focus is dependable practical support—not treating a wellness check-in as the primary service.
Personal care and routines
Respectful assistance with personal care and the established daily routines that help the person feel comfortable and oriented at home.
Mobility and everyday safety
Practical help with moving through the home, transfers and everyday activities, within the caregiver’s role and the family’s agreed plan.
Meals, hydration and groceries
Meal preparation, encouraging regular food and fluids, grocery pickup and help keeping preferred choices available.
Medication reminders only
Reminders at the agreed time only. Family Checks does not administer, organize, manage or make decisions about medication.
Appointments and errands
Support with essential errands, appointments and follow-up visits when appropriate arrangements can be made.
Companionship and updates
Calm company, reassurance and agreed family updates that help everyone understand how the transition home is going.
Immediate household help
Light household tasks directly connected to the person’s immediate care, such as dishes, tidying the care area or preparing clean bedding.
Care that feels familiar
We learn the person’s established routines, communication style, food preferences and what brings comfort, aiming for familiar caregivers whenever possible.

Hospital discharge timing and the amount of help needed at home can change unexpectedly. Families are welcome to contact Family Checks as plans develop or shift.

Every request is considered individually. Caregiver availability, the timing of the request, the person’s needs and whether support can be provided safely must all be confirmed.

Meet us before support begins.
The free, no-obligation meet-and-greet can take place in the person’s home or another mutually comfortable location. Meeting in the home can be especially helpful because the family can show us established routines, mobility needs, food preferences and how they want support to be provided. Family members may meet with us privately or include their loved one and other appropriate family members.
Family Checks provides non-medical caregiving support. Services depend on individual needs, caregiver availability and whether the requested care can be provided safely.
